国立研究開発法人 国立国際医療研究センター（NCGM）、エイズ治療・研究開発センター（ACC）の大事な使命の一つに、途上国のHIV感染者に対する共同研究を通じた診療援助があります。特に、アジアの一員としての日本は、途上国の中でもアジアの国々との共同研究が大事であると考えています。我々のカウンターパートであるベトナム拠点は、その重要な一つということになります。ベトナム以外にも、モンゴル、韓国、台湾、香港やASEANの国々など多くの病院・大学等とも共同研究を一緒に行っているサイトがあります。
One of the key mission of AIDS Clinical Center (ACC) is to support developing countries through HIV research. As being part of Asia, we have been particularly promoting collaborations with other Asian nations and the project in Vietnam is an important example. Our international research projects now expand to Mongolia, Korea, Taiwan, Hong Kong and ASEAN countries.
ベトナム拠点では、NCGMがofficialに共同研究の締結を行い、その拠点（MCC）をハノイのバクマイ病院においていますが、ACCは、さらにバクマイ病院の隣にある熱帯病病院（NHTD）にACCの研究拠点をおき活動しています。今回紹介していますNHTDでは、ベトナム保健省の倫理委員会の認可を得た後、2007年より約400名からなる患者コホートを形成しています。このコホートを用いまず行った研究は、治療を受けている患者の薬剤耐性を調べ、ベトナムでの治療の成功率を検討しました。さらに、ベトナムで流行しているHIV-1のCRF01-AEと日本の主流であるsubtype Bの薬剤耐性の比較に関する研究で、CRF01-AEのある部分の特徴的なアミノ酸の違いが薬剤耐性パターンにsubtype Bと比べ大きな違いをもたらすことを発見し報告しました。今後は、さらに治療に伴う副作用の問題や適切な治療薬の検討などアジア人に最も有効な治療法の開発に向けた臨床研究を継続し、アジアにおけるHIV診療の改良に向け貢献していきたいと思っております。
NCGM and Bach Mai Hospital (BMH) in Hanoi have tied an official agreement to enhance research collaborations and has established a project office in BMH in 2006 (NCGM-BMH Medical Collaboration Center, MCC). Simultaneously, we, ACC, has our own agreement with the National Hospital for Tropical Diseases (NHTD) since 2006 and started the cohort study with approximately 400 HIV-infected individuals in 2007. Our first study in the cohort was about the emergence of drug resistance during antiretroviral treatment (ART). We found that unique genetic patterns in CRF01-AE, which is the major HIV strain in Vietnam, cause the increased drug resistance level after failing first-line ART compared to that of subtype B, which is the most prevalent subtype in Japan. We would like to explore the best tailored treatment and monitoring methods for HIV patients in Asia through such research in Vietnam.
